Vial Of Life
MHAF’s Vial of Life Program gives first responders critical information about current medications and the health history of you and your household members. The program is simple to implement. Obtain a Vial of Life packet from MHAF, Payson Fire Department or other local first responder.
Fill out the form.
Place it in the Vial of Life bottle (the provided pill bottle). Place the bottle in your refrigerator or medicine cabinet. Place the corresponding sticker on your refrigerator or the front of your medicine cabinet.
